Transcript:
Submit TWO official copies of transcripts from each school attended (we can accept unofficial
transcripts for UW-Madison). Many institutions are starting to send official transcripts
electronically via email. These are accepted as official as long as the transcript email has been
digitally signed by the institution. Otherwise, hard-copy documents must be issued by the school
with the official seal/stamp and an official signature. International academic records must be
in the original language accompanied by an official English translation.

Personal Statement:
Your statement of purpose will be submitted with your Graduate School
online application. The statement should be no more than 2 pages in length and address the
following topics:
• Your reasons for wanting to enroll in our program.
• Your educational goals as a graduate student.
• Your research interests and career objectives. Please be as specific as possible.
• For Population Health Applicants: your proposed area of concentration (epidemiology,
health services research, social and behavioral health sciences, or clinical research).
Recommendation Letter:
Submit contact information for three letters of recommendation
from individuals who can assess your potential for success in graduate study. At least one letter
must be from a college or university faculty member with whom you have studied.
