Even if the information is listed on another school’s transcript, list every school you’ve attended since graduating from high school.

Admission is contingent upon receiving official copies of transcripts for all schools you attended.
Personal Statement:
Provide details about where you are applying and why. Include your statement of intent, personal statement, and/or proposal that reveals what you are planning to do with the fellowship, grant, job, or graduate degree for which you are applying. Information about the program/foundation is useful, especially if it’s one with which the recommender is not familiar. Your recommender needs to see your personal statement or proposal well before you send it in, in case there are changes needed. Remember, the recommender’s reputation is on the line in writing for you and mistakes or poorly organized proposals are a lousy reflection on the program, not just you.
Recommendation Letter:
Letters of Recommendation: Your 3 recommenders should submit their letters to the online system (no paper submissions to the department) shortly after the December 1oth deadline, if not before.
Personal History Statement:
Personal History Statement: Applicants must submit these essays directly into their online application. Please note that each essay should not duplicate the other. We also do not accept a personal resume or curriculum vitae as a response to either essay.The Personal History Statement is your opportunity to discuss how your personal background informs your decision to pursue a graduate degree. Please include any educational, familial, cultural, economic, or social experiences, challenges, or opportunities relevant to your academic journey; how you might contribute to social or cultural diversity within your chosen field; and/or how you might serve educationally underrepresented segments of society with your degree.
Writing Sample:
We require an 8- to 15-page writing sample (double-spaced). If you’d like to excerpt a section from a longer essay or thesis, you may include a brief summary of the rest of the argument. (You will be asked to upload it under “Additional Information” in the “Statement of Purpose & Personal History” category of the application.) Choose a strong, recent sample of your critical writing, preferably related to your proposed research interests.
